• ALL CAMPS - Please do NOT send any nut products or products containing nut or nut oils with your camper. Several of our campers have life-threatening nut allergies.

• Summer Registration Form (This form can also be used for Adult/Teen Workshops and for private music lessons. Maximum summer registration fee is $15 per family per summer.)

• Camper's Immunization Records & copy of last physical (your doctor may submit this using his/her own form)

Creative Arts Camps are licensed by the state; therefore, we are required to collect your child's immunization records and receive a copy of the most current physical. New this year for the Art Camps in Reading, we must also receive the TB Questionnaire that must be signed by a physician. Thank you for helping us comply with these rules and regulations.
